Những tác động của nhổ răng chỉnh nha

Ngoài những yếu tố như không khả hồi (irreversible), răng cần nhổ có thể là răng thật còn nguyên vẹn thì điều quan trọng nhất là nhổ răng sẽ ảnh hưởng đến khuôn mặt của bệnh nhân.

Vì vậy, bác sĩ chỉnh nha luôn phải cân nhắc có nên nhổ răng hay không dựa trên việc dự đoán được khuôn mặt của bệnh nhân sau khi kết thúc kế hoạch điều trị và sau 10 - 20 năm sau đó nữa!

Do đó tất cả những trường hợp nhổ răng khi chỉnh nha đều được coi là những ca khó và cần nhiều thời gian để hoàn thành kế hoạch điều trị hơn.

Trẻ em ít nhổ răng hơn người lớn

Đối với trẻ em, ở độ tuổi dậy thì sẽ có sự tăng trưởng và phát triển (growth) rất nhanh, khuôn mặt và xương hàm sẽ phát triển nên nếu quyết định nhổ răng quá sớm sẽ ảnh hưởng không tốt, thậm chí là tiêu cực đến khuôn mặt của trẻ sau này.

Vì thế, tỉ lệ nhổ răng để chỉnh nha ở trẻ em ngày chỉ khoảng 5-10%. Và càng đưa trẻ đến gặp BS chỉnh nha sớm (8 tuổi là độ tuổi nên gặp bác sĩ chỉnh nha đầu tiên, sau khi 4 răng cửa vĩnh viễn hàm trên đã mọc đầy đủ), càng ít khả năng trẻ phải nhổ răng để điều trị chỉnh nha.

Còn ở người lớn, sẽ không còn sự phát triển và thay đổi khuôn mặt cũng như xương hàm. Vì vậy tỉ lệ nhổ răng để điều trị chỉnh nha sẽ cao hơn.

Ngoài ưu điểm hạn chế đau và khó chịu khi chỉnh nha, kỹ thuật chỉnh lực nhẹ hiện nay còn tạo xương và mô mềm (nướu) khi răng di chuyển, giúp hạn chế nhổ răng hơn. Chỉ những trường hợp có khuôn mặt với xương hàm dưới hoặc xương hàm trên đưa ra trước quá nhiều thì sẽ chỉ định nhổ răng. Đặc điểm này cũng giúp hạn chế những trường hợp cần phẫu thuật để chỉnh nha (Orthognathic Surgery).

Như vậy nhổ răng hay không nhổ răng trong điều trị chỉnh nha vẫn còn là một chủ đề gây nhiều bàn cãi giữa các bác sĩ chỉnh nha. Tuy vậy, với kỹ thuật sử dụng lực nhẹ để tạo xương và mô mềm cùng lúc với di chuyển răng ngày nay, tỉ lệ nhổ răng ngày càng ít đi, chỉ còn 20% tổng số ca chỉnh nha (trước đây là 80%). Trẻ em có tỉ lệ nhổ răng ít hơn nhiều so với người lớn.
