Nhiễm nấm vì ngoáy tai nhiều

(Dân trí) - Những người thường hay đi cắt tóc, gội đầu, ngoáy tai không may bị xây xước, tổn thương, chảy máu sẽ có nguy cơ viêm ống tai ngoài do nhiễm nấm.

Bệnh viêm ống tai ngoài do bị nhiễm nấm (otomycosis) có thể tiến triển mạn tính hoặc bán cấp tính và thường đi theo sau viêm tai do nhiễm khuẩn; nó cũng có thể xảy ra sau khi bị chấn thương. Bệnh phổ biến ở khắp mọi nơi, nhất là những vùng nhiệt đới nóng ẩm.

Nấm gây bệnh viêm ống tai ngoài gồm một số loài nấm sợi và nấm men như Aspergillus, Mucor, Penicillium, Rhizopus, Candida... Các loài nấm này thường sống hoại sinh ở trong đất.

Tổn thương do nấm gây nên thường ở ống tai ngoài hoặc vành tai. Trong thời gian đầu, người bệnh không để ý vì chưa có biểu hiện gì đặc biệt; chỉ bị hơi viêm và hơi khó chịu. Sau đó, bệnh trở nên mạn tính, tổn thương dần dần lan rộng, chất mủ có thể xuất hiện do nhiễm khuẩn thứ phát; có một số vảy, mảnh vụn hình thành ở trong ống tai ngoài và vành tai. Người bệnh có triệu chứng cơ năng rất ngứa, cảm giác khó chịu, làm ảnh hưởng đến khả năng nghe.

Chẩn đoán bệnh viêm ống tai ngoài do nhiễm nấm thường dùng phương pháp soi trực tiếp bằng cách cạo vảy da ở ống tai ngoài, vành tai cho vào dung dịch KOH 20% để xét nghiệm dưới kính hiển vi phát hiện sợi nấm hay tế bào nấm men. Cũng có thể sử dụng phương pháp nuôi cấy bằng cách dùng vảy da ở tai và cấy vào môi trường Sabouraud ở nhiệt độ 28^oC, để trong thời gian từ 1 đến 2 tuần sẽ thấy nấm mọc. Kiểm tra đại thể và vi thể để xác định nấm.

Nguyên tắc điều trị bệnh viêm ống tai ngoài do nhiễm nấm là trước khi bôi thuốc cần dùng bông cồn hay bông tẩm dung dịch Barrow làm sạch ống tai, vành tai. Sau đó bôi thuốc chống nấm tại chỗ như nizoral, fungal, Whitfield. Có thể kết hợp điều trị với thuốc chống nấm ketoconazol hay itraconazol uống.
