Bác sĩ bất ngờ khi bệnh nhân mổ thanh quản trở nặng
"Bản thân bác sĩ cũng không lường hết được, vì thủ
thuật lần này giống lần mổ trước, chỉ là bấm một nang nước nhỏ", lãnh
đạo Bệnh viện Đa khoa Hà Nội lý giải về trường hợp bệnh nhân 51 tuổi, ở
Bắc Ninh, nguy kịch sau mổ thanh quản.

"Mới mở miệng, xịt thuốc tê thì bệnh nhân đã ngưng thở, co thắt thanh quản. Không khí không vào được phổi, thiếu ôxy đến các cơ quan, nhất là não. Trong khi đó với một nang nước nhỏ ở thanh quản, đặt dụng cụ đúng chỗ, chọc nước ra thì chỉ mất 2 giây", phó giáo sư Nguyễn Hoàng Sơn, Giám đốc phụ trách chuyên môn Bệnh viện Đa khoa Hà Nội nói.
Theo ông, có nhiều nguyên nhân dẫn đến co thắt thanh quản như: viêm nhiễm do vi trùng, siêu vi trùng, u thanh quản, chấn thương thanh quản, dị vật hóc... Trong trường hợp bệnh nhân Tưởng, thì ông nghĩ nhiều đến khả năng do thuốc xịt của bệnh nhân.

Trong khi đó, bác sĩ Vũ Tuấn Anh, Trưởng phòng mổ, Bệnh viện Đa khoa Hà Nội thì cho rằng, bà Tưởng được đưa vào phòng mổ, xịt thuốc mê 3 lần (giống loại dùng cho lần mổ đầu tiên). Sau đó bệnh nhân vẫn bình thường, vì thế có thể loại trừ khả năng phản ứng do thuốc xịt gây tê tại chỗ. Một lúc sau khi đặt càng banh miệng, bệnh nhân mới có biểu hiện bất thường.
"Vấn đề đúng hay sai, vì sao tai biến sẽ do cơ quan chuyên môn đánh giá, còn về quy trình thì chúng tôi làm đúng. Chúng tôi đã gửi báo cáo lên thanh tra Sở, công an. Đồng thời thống nhất với gia đình hỗ trợ toàn bộ tiền điều trị của bệnh nhân tại bệnh viện này cũng như tại Bệnh viện Việt Đức", ông Sơn nói.
Ngày 30/10, bà Trần Thị Tưởng (Từ Sơn, Bắc Ninh) đến Bệnh viện Đa khoa Hà Nội để lấy nang ở dây thanh quản. Mổ xong, bệnh nhân khỏe mạnh ra về. Ngày 8/11 bệnh nhân đến khám lại vì thấy nói bị khàn, PGS Nguyễn Thị Hoài An khám thấy có một nang nước nhỏ ở dây thanh quản nên cho lên phòng mổ để bấm nang nước.
Vào phòng mổ bệnh nhân lên cơn co thắt nguy kịch. Đêm đó, bà được chuyển sang Bệnh viện Việt Đức điều trị tiếp. Hiện tại bà Tưởng chưa tỉnh, vẫn thở máy, mạch, huyết áp vẫn còn. Bệnh viện Đa khoa Hà Nội là bệnh viện tư, mới đi vào hoạt động được 2 năm.
