Vì sao phụ nữ mang thai cần Folate?

Theo nghiên cứu, 63% phụ nữ trong độ tuổi sinh đẻ tại Việt Nam tham gia cuộc khảo sát có nồng độ Folate hồng cầu thấp dưới mức tối ưu (>=905nmol/L) để phòng ngừa khiếm khuyết ống thần kinh ở thai nhi.

Thưa bác sĩ, bác sĩ có thể cho biết Folate có vai trò như thế nào đối với sức khỏe của phụ nữ mang thai và sự phát triển của thai nhi?

Folate (Axít Folic) hay còn gọi là vitamin B9 có vai trò rất quan trọng đối với sự phát triển và phân chia của mọi tế bào trong cơ thể. Chính vì thế, thiếu Folate có thể ảnh hưởng đến khả năng chống lại các bệnh nhiễm trùng của hệ thống miễn dịch. Đặc biệt, phụ nữ mang thai có nguy cơ thiếu Folate cao gấp 10 lần so với phụ nữ không mang thai nên dễ gây thiếu máu, nguy cơ sẩy thai cao, sinh non, sinh con nhẹ cân. Đối với thai nhi, việc thiếu Folate có thể gây nên những dị tật về ống thần kinh, tăng nguy cơ khiếm khuyết ống thần kinh ở thai nhi.

Khiếm khuyết ống thần kinh là gì và ảnh hưởng như thế nào đến sự phát triển của thai nhi, thưa bác sĩ?

Khiếm khuyết ống thần kinh xảy ra do sự hình thành không bình thường của ống thần kinh trong thời kỳ bào thai như nứt ống thần kinh, thiếu một phần não hoặc thoát vị não. Ống thần kinh đóng không hoàn toàn là nguyên nhân dẫn đến nhiều bệnh lý của cơ thể như bại liệt, não úng thủy… Đa số các trẻ bị khiếm khuyết này thường không sống được. Một số nghiên cứu cho thấy có tới 94% trẻ sơ sinh mắc các khiếm khuyết này bị tử vong trong vòng 24 giờ sau sinh.

Theo bác sĩ, phụ nữ mang thai cần bổ sung lượng Folate bao nhiêu là đủ và bổ sung bằng cách nào?

Theo khuyến nghị của Bộ Y tế năm 2007, phụ nữ trong độ tuổi sinh đẻ cần 400mcg Folate mỗi ngày. Khi mang thai, nhu cầu Folate tăng lên nhanh chóng để đáp ứng cho sự tăng trưởng của thai nhi và thay đổi của thai phụ, chính vì vậy, phụ nữ có thai cần được cung cấp 600mcg Folate mỗi ngày.

Một nghiên cứu chính thức gần đây đã chỉ ra rằng, không có chứng cứ về sự thiếu hụt folate và vitamin B12 ở nhóm phụ nữ Việt Nam được nghiên cứu. Tuy nhiên, 3 trong 5 phụ nữ tham gia nghiên cứu có nồng độ folate dưới mức tối ưu, và như vậy làm tăng nguy cơ khuyết tật ống thần kinh (NTD). Tuy nhiên, vẫn còn khả năng giảm tỷ lệ khuyết tật ống thần kinh (NTD) và phụ nữ có thể bổ sung các nguồn axit folic trong thời gian đầu thai kì từ các loại thực phẩm bổ sung và thực phẩm tăng cường.

Có thể bổ sung Folate thông qua thực đơn hàng ngày, tăng cường sử dụng các thực phẩm giàu Folate như gan, lòng đỏ trứng gà, trứng vịt, vừng, lạc, các loại đậu (đậu đũa, đậu xanh, đậu tương, đậu Hà Lan), các loại rau (rau muống, cải xanh, cải cúc, súp lơ xanh)... Ngoài ra, việc dùng thêm một số thực phẩm bổ sung hàm lượng Folate cao như sữa, bánh qui, bột mì cũng làm tăng lượng Folate cho cơ thể. Thai phụ cũng có thể uống bổ sung viên Folate, viên sắt và acid folic theo phác đồ của Bộ Y tế và Tổ chức Y tế Thế giới.
